Call the SelectHealth Member Services team at 1-866-469-7774 (TTY: 711) to find out if you qualify. They can check your eligibility Monday — Friday, 8 am — 6 pm.

If you’re eligible, SelectHealth can help you enroll.
SelectHealth is not your average Medicaid health plan. With SelectHealth, you get all the benefits of Medicaid, like medical, dental, vision, and behavioral (mental health) coverage, plus extra support from a Care Manager to keep you healthy. You can learn more about SelectHealth benefits here.
If you do not have your Medicaid card or Medicaid number, you will be asked to provide the following: your first name, last name, date of birth, and Social Security number.
Because the plan is classified as a “Special Needs Plan”, eligible people can join SelectHealth at any time.

If you enrolled in Medicaid through the Department of Social Services (HRA), call NY Medicaid Choice at 1-800-505-5678 and tell them you want to switch to SelectHealth.

If you enrolled in Medicaid through NY State of Health Marketplace (https://nystateofhealth.ny.gov/): please call 1-855-355-5777 and tell them you want to switch to SelectHealth.
To keep your coverage active, you must re-certify your Medicaid every year.

If you enrolled in Medicaid through NY Medicaid Choice (HRA): call 1-800-505-5678 or access the ACCESS HRA mobile app or website.

For people who have been classified as having a disability by HRA, you can re-certify your Medicaid by completing forms you get in the mail from New York State Medicaid. Check your mailbox often.

Yes, if you already have a NY State of Health Marketplace account, you can enroll online at https://nystateofhealth.ny.gov/. Sign into your account and pick SelectHealth by VNS Health on the plan selection page.
